Please choose the appropriate form(s), print and fill out. Completed forms can be dropped off or mailed to the Clatsop County Manager's Office, Volunteer Program, 800 Exchange St., Suite 410, Astoria, OR 97103.

Some volunteer positions require two different application forms.

General Volunteer Application Form

Citizen Committee Application Form  (you do not need to  fill out the General Volunteer Application Form if you are interested in serving on a Citizen Committee)

Animal Shelter Volunteer Application Form 

Animal Shelter Waiver Form (in addition to Animal Shelter application)

Animal Shelter Youth Transportation (in addition to Animial Shelter application, if 17 or younger)

CADY Juvenile Mentoring Program  (in addition to the general form)
